Early Performance of Accountable Care Organizations in Medicare

In the Medicare Shared Savings Program (MSSP), accountable care organizations (ACOs) have financial incentives to lower spending and improve quality. In this article they used quasi-experimental methods to assess the early performance of MSSP and ACOs. In the conclusions they found that the first full year of contracts MSSP was associated with reductions in Medicare Early pass between the 2012 participants but not among the 2013 participants. Economies higher in primary care groups independent groups in integrated hospitals. 

Empowering Better End-of-Life Dementia Care (EMBED-Care): A mixed methods protocol to achieve integrated person-centred care across settings

It is known that the number of people with dementia continues to increase in the world. We also know that people with dementia live to advanced stages of the disease, so they will require comprehensive palliative care. The aim objetive of this work, is to promote a “step change” in care for people with dementia and their carers to enable delivery of timely person-centred care, to identify unmet needs and improve outcomes, including comfort and qualitive of life, towards end of life. Each of six-interdependent work streams (WSs) will deliver new knowledge and sequentially build a complex intervention to improve palliative dementia care.

The use of the performance management (PM) system is highly contested by public servants in South Africa, although its value as essential to the appraisal and management of staff is undeniable. Before considering the integration of a system or parts of a health system, a diagnosis must be made to assess how segmented it is.
